Action Step(s)
- Form a multi-disciplinary quality improvement team dedicated to providing high quality, person-centered contraceptive care.

Set clear quality improvement aims that seek to improve the CHC’s performance measure data (i.e., baseline PCCC measure and eCQM data) and result in sustainable change; use these aims to communicate goals and expectations to team members.

Establish measures, including process measures and other data points, to determine whether operational changes implemented lead to improvement. Team members often provide valuable and creative ideas for additional data sources for this step.

Select, test, and implement changes using the PDSA model.
