QI Process

To support the implementation of strategies and evidence-based interventions during each Action Period, CHCs are welcome to use any model for improvement they would like to use, and we encourage CHCs to utilize their usual QI processes in this project. We are providing this Model for Improvement as an option for CHCs to use. For those who would like a model to use, you may want to use this model for improvement tool for change management.1 The model has two parts:

Three fundamental questions, which can be answered in any order:

  • What are we trying to accomplish?

  • How will we know that change is an improvement?

  • What changes can we make that will result in improvement?

Plan-Do-Study-Act (PDSA) cycles aimed at testing changes in real work settings.

In addition, there are seven corresponding processes that guide improvement (as shown below in Figure 1). The website is organized into sections that summarize each of the corresponding processes and their relationship to the model.
