Key to any QI process is the identification of opportunities for positive change. CHCs may use the self-assessment included on the website (see "Assessment" tab to the left) as part of their diagnostic process. This tool aims to evaluate the extent to which each improvement team members – from their unique perspective – believes the CHC has the optimal foundation and infrastructure for patient-centered contraceptive care services delivery.
This assessment provides insight into areas in which there are opportunities for impact, particularly when considered in combination with baseline PCCC measure and eCQM scores, both overall and in relation to one another.
